Former ESFNA Executive Committee members and their spouses are eligible for VIP passes. Complete the form below with a recent passport-standard photo to request your ID. This portal form replaces emailing secretary@esfna.org.

      1. Passport-standard photograph (i.e. no hats, sunglasses or side pictures )

      3. Minimum resolution: 300px x 360px
      4. File size less than 5 MB
